Denver Juvenile Court
The Denver Juvenile Court is a separate trial court within Colorado’s Second Judicial District, serving all of the City and County of Denver. Unlike other judicial districts in Colorado, the Second Judicial District is not a combined court and is comprised of four separate courts: County Court, District Court, Probate Court, and Juvenile Court.
Denver Juvenile Court exists to promote stability and well-being for children and families, to improve safety for children and the community, to ensure that children stay connected to their culture, community and support systems, and to provide fair and equitable access to justice for families.
Denver Juvenile Court is led by a Presiding Judge and is supported by two Juvenile Court Judges, two Juvenile Court Magistrates, a Court Executive, a Clerk of Court, a Family Court Facilitator, and a Chief Probation Officer. Juvenile Court Judges serve six-year terms, consistent with District Court Judges, and are subject to voter retention within the judicial district. Denver Juvenile Court is located at the Lindsey-Flanigan Courthouse, which also houses Denver Criminal Courts, as well as District and County Court proceedings.

To file to establish paternity, the following forms are required:
- District Case Management Order
- Case Information Sheet – JDF 1000,
- Petition for Paternity – JDF 1501,
- Summons for Paternity – JDF 1502, and
- Final Order of Paternity – JDF 1516.
**More forms will be required after beginning your Paternity case.**
To file to disclaim paternity, the following forms are required:
- Case Information Sheet – JDF 1000,
- Petition to Disclaim Paternity – JDF 1514,
- Summons to Disclaim Paternity – JDF 1515, and
- Final Order of Paternity – JDF 1516.
**More forms will be required after beginning your Paternity case.**
To file to modify child support, the following forms are required:
- Verified Motion to Modify Child Support – JDF 1403,
- Sworn Financial Statement – JDF 1111, and
- Order Re: Modification of Child Support – JDF 1405.
To file for custody orders, the following forms are required:
- Case Information Sheet – JDF 1000,
- Petition for Allocation of Parental Responsibilities – JDF 1413,
- Parenting Plan – JDF 1113,
- Certificate of Service – JDF 1313, and
- Order for Allocation of Parental Responsibilities – JDF 1422.
To file to modify parenting time, the following forms are required:
- Motion to Modify/Restrict Parenting Time – JDF 1406,
- Order Re: Modification/Restriction of Parenting Time – JDF 1424.
