Forms Development Process
Form update projects come from statutory changes and suggestions from Judicial Staff and the public. While the process can vary from project to project, in general:
Minor Corrections
- These changes include typos, broken links, wrong citations (include the correct citation), grammatical/spelling errors, etc.
- Submit changes by email with the form number to Forms@judicial.state.co.us.
- Preference changes, new forms, and major corrections/changes follow the phases below:
Phase 1 - Drafting & Submission
- Make the changes you're suggesting in the Word (docx) version of the form with the tracked changes feature (changes appear in red).
- To get a version of the form in Word, email Forms@judicial.state.co.us.
- Also, in the Word file, add a comment stating the reason for the change. Cite to statute, rule, Chief Justice Directive (CJD), or BBP.
- Complete JDF 1 - Forms Change Request.
- List the forms (with JDF numbers) and give a summary of changes.
- You must seek endorsements for the project from local leadership and a statewide group.
- The dropdown form fields in the endorsement section lists some of the possible endorsement options.
- Submit the completed JDF 1 and the Word files by email to Forms@judicial.state.co.us.
Phase 2 - Drafting and Review
- JDF Projects are triaged and reviewed by one of the JDF Subject Matter Expert Workgroups. Standing workgroups include:
- Civil
- Criminal & Delinquency
- Family: Domestic & Juvenile
- Housing
- Probate Forms are passed on to the Probate Rules Committee.
- Probation Forms are passed on to the Division of Probation Services at SCAO.
Phase 3 - Editing
- Content is edited for formatting and legal accuracy.
Phase 4 - Approval
- Changes are submitted to the Colorado Supreme Court for approval.
Phase 5 - Remediation, Publication, & Translation
- Form fields are added.
- The form is coded to meet accessibility requirements.
- The form is published.
- The form is submitted for translation (as needed).

Can't view the form fields in your browser?
- Make sure your browser has a PDF Reader plugin and JavaScript enabled.
- Otherwise, download/save the PDF file to your phone or computer. Then open with a PDF reader application.
Tips for successful form searching
- When searching a known form number, place a space in between the JDF and the number(s), or just the numbers.
Example: for form JDF1220
Enter in the Keyword field: "JDF 1220"; or "1220" - If any of these forms are updated, the form links may change.
- For best results, download the form and use a PDF reader to complete.
