Name Restoration
Request a name restoration to your prior name (before union/marriage) in the Petition (JDF 1011) or Response (JDF 1015).
Or:
- File JDF 1824 - Motion for Name Restoration in the court where the decree was entered. And
- File JDF 1825 - Order. (caption only)
Filing Fee
- There is no filing fee if you file for a name restoration within 60 days after the decree has been signed.
